Advocacy for older individuals can take many forms, such as lobbying for policies that benefit older adults, raising awareness about the issues facing older adults, providing legal assistance, and offering support services such as counseling and healthcare. Advocates can also help to connect older adults with community resources, such as housing, transportation, and social activities. 

  • Explore entitlement programs and resources in the community.

  • Act as a liaison between the client and management, medical team, and social work staff at facilities.

  • Connect client with appropriate resources in the community by way of calling, application and follow up.

  • Liaison between the client or their authorized representative with family members, legal team, medical team and financial team in the community.

  • Work with Article 81 Guardian in NYS and Conservators in CT to ensure, their wards receive the care they are entitled to.

  • Accompany the client on ER visits 24/7

  • Schedule regular visits to nursing homes or assisted living.
